The following reflects Brookmires' approved Notice of Privacy Practices regarding protected health information (PHI), for clients and individuals whose health information we may handle in the course of insurance services such as Medicare guidance.

We may use and disclose protected health information to carry out functions including: enrollment; claims payment and auditing; quoting; underwriting activities; case issuance; customer service, including identification card and demographic change requests; compliance and oversight audits; reporting privacy violations to appropriate authorities; responding to public health and safety authorities; complying with law enforcement, judicial, and administrative proceedings where required; and investigating fraud. We do not sell protected health information.
Your privacy rights. As an individual whose health information we may hold, you have the right to:
- Access — request to review or receive a copy of your protected health information. We will respond to a written request within 30 days.
- Accounting of disclosures — request a list of certain disclosures of your health information made in the past six years for purposes other than treatment, payment, or plan operations.
- Restriction — request a limit on how your health information is used or disclosed. We are not required to agree if the restriction would interfere with treatment, payment, or operations.
- Alternate communications — request that we contact you using a different address or method, for example if standard communication could pose a safety risk to you.
- Amendment — request a correction to health information you believe is inaccurate or incomplete. We will respond within 60 days.
- File a complaint — you may file a privacy complaint with us directly, or with the Secretary of the U.S. Department of Health and Human Services, without fear of retaliation.

In the course of providing insurance and financial services, we may collect nonpublic personal information about you from applications, transactions, and other communications. We may share this information with insurance carriers, service providers, and other parties as necessary to provide the services you've requested, and as permitted or required by law. We do not share your information with unaffiliated third parties for their own marketing purposes without your consent. You have the right to opt out of certain information sharing — contact us using the information below to do so.
